Insurance
We do participate and file with several insurance plans, however we do not courtesy verify your insurance prior to scheduling. If you are an established patient with new insurance, this is the patient information and insurance form. (Google Drive link.) It does have to be filled out, signed and returned to the office by the business day before your appointment at the latest, or your appointment will be filed as “self-pay.”
We participate with the following companies, at the PPO level:
- Aetna Behavioral Health
- Blue Cross and Blue Shield of Texas
- Evernorth Behavioral Health (formerly Cigna Behavioral Health)
- Humana Behavioral Health
- Magellan Health Services
- OptumHealth (a branch of United HealthCare)
As well as several other insurance plans that utilize the CAQH Universal Provider Datasource. Always contact your insurance if you are unsure if a provider is covered under your plan.
